About this audit resource
This resource provides two distinct ways to test delivery against the Client Pathway Standards and Practice requirements.
- Client journey audit: follow one selected client record through the relevant parts of the model, with a jump link to the selected area. Save a separate copy of the workbook for each client where several journeys are sampled.
- Thematic audit: choose an area of service, then select from only the audit checks belonging to that area and test the chosen requirement across every eligible client record or an agreed sample.

The workbook opens with a clear choice between the two audit routes. It contains one reusable client journey form, a separate thematic audit form with area-filtered audit checks, all 245 checks covering 3A–3F and the cross-pathway checks, and an automatic findings summary.
Before an audit begins
- Agree the audit purpose, scope, sample, responsible auditor and reporting period.
- Choose the client journey or thematic route before entering findings.
- Record a reproducible selection method so the strongest records cannot be chosen deliberately or inadvertently.
- Use a unique local Audit ID and the Lamplight/client-record Client ID. Do not enter a client name.
- Confirm where the completed workbook and agreed actions will be stored and who may access them.
Recording findings
- Yes: the sampled evidence demonstrates the requirement.
- Partly met: some evidence is present but a material element is incomplete, inconsistent or unclear.
- No: the requirement is absent or the sampled evidence demonstrates a departure from it.
- Not applicable: the requirement genuinely does not apply to the agreed sample. Record the reason where it may not be self-evident.
For every Partly met or No finding, record the gap, corrective action, responsible function, due date and action status.
Information and storage controls
Do not copy any case information into the workbook. Use Lamplight as the client record and record only where the evidence was found.
Completed audit workbooks may contain governance and case-quality information. Store and share them through the Delivery Site’s agreed controlled organisational route, with access limited to the people who need it.
Analysis and follow-up
Use the workbook summary to review the completed journey and thematic forms. Interpret percentages with the number of completed checks and the audit scope; unlike samples should not be treated as directly comparable.
Audit completion is not the end of the process. Corrective actions must be assigned, monitored and closed, and repeated gaps should inform supervision, training, governance and model-fidelity review.
